By signing this agreement, I acknowledge that participation in virtual reality activities at Zack’s VR Arena involves inherent risks, including but not limited to dizziness, motion sickness, loss of balance, collisions, falls, physical injury, emotional stress, equipment malfunction, and other unforeseen risks that may result in injury, illness, property damage, or death.
I voluntarily choose to participate and fully assume all risks associated with participation in VR activities, arcade games, simulators, and any related activities within Zack’s VR Arena.
I certify that:
I agree to follow all posted rules, staff instructions, and safety guidelines. I understand that failure to follow safety instructions may result in removal from the facility without refund.
To the fullest extent permitted by law, I release and hold harmless Zack’s VR Arena, its owners, employees, contractors, affiliates, and partners from any and all claims, liabilities, damages, injuries, losses, or expenses arising from participation in activities at the facility, including those caused by ordinary negligence.
I understand that Zack’s VR Arena is not responsible for lost, stolen, or damaged personal property.
I grant permission for Zack’s VR Arena to use photos or videos taken during participation for promotional or marketing purposes unless I notify staff in writing otherwise.
If the participant is under 18 years old, a parent or legal guardian must sign below and accepts full responsibility for the minor participant.
By signing below, I acknowledge that I have read, understood, and voluntarily agree to this Waiver and Release of Liability.
